About the Job
The Registered Nurse/Registered Psychiatric Nurse:
• Functions as a member of the larger Hospitals & Communities Integrated Services (HCIS) or Tertiary Mental Health Services multi-disciplinary care team
• Ensures that persons with serious severe and persistent mental health problems, and/or problematic substance use and/or development disability issues who may or may not be in crisis have access to timely, responsive, evidence based treatment and clinical supports across the continuum of available services
• Provides assessment, treatment, case management, community support, and as required, urgent response to clients who may also have serious functional impairment in the domains of physical health and personal, family, financial, occupational, and social life
• Functions as a primary clinician utilizing a variety of specialized treatment modalities/therapies for individuals, groups of clients and/or their families

Intensive Case Management teams help clients with severe mental health and/or substance use concerns through team-based care coordination. Services may include Substance Use Counselling & Treatment Services plus housing assistance, primary care & outreach.

Intensive case management services are a key part of the continuum of mental health services and supports for people with serious mental illness. Intensive case management promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support as needed by the consumer. The direct involvement of the consumer and the development of a caring, supportive relationship between the case manager and the consumer are integral components of the intensive case management process. Intensive case management is responsive to consumers’ multiple and changing needs, and plays a pivotal role in coordinating required services from across the mental health system as well as other service systems (i.e., criminal justice, developmental services, and addictions).

Case managers fulfil a vital function for consumers by working with them to realize personal recovery goals. Case managers work to build a trusting and productive relationship with the consumer and to provide the support and resources that the consumer needs to achieve goals, stabilize his/her life and improve his/her quality of life.

This is a Permanent Full Time (1.0) opportunity.
Shifts of work will be scheduled days from 08:30 to 16:30 hours.

Qualifications
• Graduation from an approved school of nursing.
• Two years’ recent related MHSU experience in the assigned clinical area(s) (such as Counselling & Treatment Services; Treatment, Support & Recovery Services; Crisis Response; Seniors MHSU Services; Substance Use; Developmental Disability Mental Health Services (DDMHS), etc.) or an equivalent combination of education, training and experience.
• Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
• Current valid B.C. driver’s license.
