Role and Facility Information
In this role, you will work in a multidisciplinary PCN Clinic alongside a large team of Physicians, Nurse Practitioners, Pediatricians, Internal Medicine an RN to deliver best in class primary care. This role can be a hybrid position, allowing you to securely work from home using their paperless cloud-based system. Pharmacist in house.
Responsibilities:
• In clinic and/or telemedicine care for patients.
• Maintain accurate and up-to-date electronic medical records.
• Collaborate with other healthcare providers within our team to ensure continuity of care and seamless patient experience.
• Adhere to all privacy and confidentiality regulations as required by law.
- Provide primary care to patients of all ages, focusing on health promotion and disease prevention
- Conduct physical exams and patient assessments
- Diagnose and treat common acute illnesses and injuries
- Manage chronic health conditions such as diabetes, hypertension, and asthma
- Prescribe medications and treatments
- Educate patients on maintaining a healthy lifestyle and disease management
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care
Skills
- Experience in medical office settings, hospitals, or home care environments
- Proficiency in patient care across various age groups including children, seniors, and individuals with complex health needs
- Knowledge Women’s Health
- Strong communication and interpersonal skills
- Ability to work independently as well as part of a multidisciplinary team

Schedule:
