Clinic Hours: Mon-Fri 8:30 - 16:30

Payment Model Options: Positions are available under LFP, FFS and NTP PCN (salary) contracts

• Physician Lead
• 3 Full-time family physicians
• 3 Full-time MOAs
• PCN Allied Care Professionals (3 Chronic Disease Management Registered Nurses; 1 Social Worker; 3 Clinical Counsellors; 1 Registered Dietitian; 2 Physiotherapists; 1 Occupational Therapist; 1 Clinical Pharmacist; 1 Frail Seniors Team Registered Nurse; 1 Frail Seniors Team Occupational Therapist; 1 Frail Seniors Team Social Worker; 1 Community Link Worker; 1 Administrative Coordinator; & Chronic Disease Group
