Seeking a Career Locum, Permanent Float to support this clinic location's mandarin and cantonese community. This Career Locum will help with vacation leave coverage and patient overflow via the LFP payment model.

Open to considering the following practice arrangements to fill this family physician (FP) vacancy:
1. Locum-to-Permanent (FPs seeking to trial various practice settings prior to securing a clinical home & beginning to build a patient panel)
2. Career Locum, Permanent Float (FPs seeking roles that do not require building a patient panel – i.e., providing regular/scheduled walk-in, patient overflow or vacation coverage)

Payment Model Option:
- Longitudinal Family Physician (LFP) Contracts

Locum Billing Model Options Available:
- Fee-Sharing Payment Model
- Assignment of Payment Model

Billing/Fee-sharing Payment Split: 75/25

Schedule Flexibility: Flexible hours - open to considering partial or reduced hours

EMR Expectations: Software training & support available on-site

• PCN Allied Care Professionals (3 Chronic Disease Management Registered Nurses; 1 Social Worker; 3 Clinical Counsellors; 1 Registered Dietitian; 2 Physiotherapists; 1 Occupational Therapist; 1 Clinical Pharmacist; 1 Frail Seniors Team Registered Nurse; 1 Frail Seniors Team Occupational Therapist; 1 Frail Seniors Team Social Worker; 1 Community Link Worker; 1 Administrative Coordinator; & Chronic Disease Group
