Role and Facility Information

The Primary Care Access Clinic (PCAC) supports the WRSS Division of Family Practice’s mission to improve the overall health of residents in our community while developing patient-centred primary care programs in our area. The clinic was developed in partnership between the Division and Fraser Health Authority. This clinic is operated by nurse practitioners and physicians, with support from MOAs, and is a multidisciplinary clinic which provides transitional primary care for unattached patients in our community.

Our Mission at the PCAC is to:

• support family physicians who provide in-hospital care to unattached patients by offering immediate post-discharge follow-up care;

• support emergency room practitioners by providing a place where unattached patients – who don’t require a hospital stay – can be referred for follow-up care; and

• provide ongoing primary care for complex clients, including those with mental health and addiction problems, who would be optimally managed in a multidisciplinary setting.

Our Patients

The clinic temporarily supports unattached patients who require time-sensitive follow-up care upon discharge from Peace Arch Hospital. Unattached patients who require ongoing care will be connected to appropriate community services, such as home health, mental health and addictions, and complex case management. The clinic will also provide long-term care for patients who are difficult to attach to traditional primary care practices, who are vulnerable and or have complex needs. More time is spent with patients (30 or more minutes).

Our Practice

Family physicians and nurse practitioners work in collaboration and the clinic has a dedicated Medical Office Assistant (MOA). The PCAC team continues to play a pivotal role in triaging patients and referring appropriate patients to new physicians and to those physicians accepting new patients.

The PCAC is part of our Primary Care Networks (PCN). The network focuses on team-based primary care services and enhancing capacity and patient access. PCNs bring together health authorities, physicians, nurse practitioners, nurses, allied health professionals, and community providers to ensure that all members of the community receive the primary care they require.

The Clinic is in on the campus of Peace Arch Hospital (which has 160 Beds). The area has a population of approximately 119,000; a 15% growth in the last 5 years.

Walk or cycle through forest trails, enjoy water sports at both White Rock and Crescent Beach. Located five minutes from the US border and 45 minutes from downtown Vancouver.
