Bring your clinical expertise and passion for providing quality, compassionate care to a challenging and rewarding role as a Nurse Practitioner, Primary Care Network to the Boundary Medical Clinic in the city of Grand Forks.
Work where you play in this four-season lifestyle opportunity. You will be joining a multi-practitioner clinic that has provided care to the Grand Forks area for over 40 years. Situated in the walkable downtown core of Grand Forks, BC, you will be practicing rural family medicine an hour away from two world-class ski resorts, Red Mountain and Whitewater, and even closer to golf courses, lakes, hiking and biking trails, and rural campsites.
Boundary Medical clinic is part of the Kootenay Boundary Primary Care Network (KB PCN), which is a partnership between the Ministry of Health, Interior Health, the Kootenay Boundary Division of Family Practice, and Aboriginal Partners. All aspects of the clinic are aligned with delivery of culturally competent care.
You’ll be joining a collegial team of practitioners and knowledgeable, experienced staff. The clinic is in a modern, purpose-built building with 4 offices and attached exam rooms, a well-equipped procedure room, and a staff lounge. The clinic uses Profile EMR. Pre-natal care only (no deliveries in the community). Interest in geriatric care is an asset. The multidisciplinary team includes a primary care Nurse and a Social Worker, as well as a foot care clinic.
The Nurse Practitioner, Primary Care Network, will practice autonomously and interdependently as part of a cohesive team where each provider has their own unique role and scope in the provision of patient care. The Nurse Practitioner will work in partnership with a group of Family Physicians and with the guidance of a Medical Director to provide clinical care and ensure full coverage to clinic patients.
The Nurse Practitioner will deliver the full scope of primary health care services for their panel of patients (rural panel goal is 800 patients pro-rated for FTE), including but not limited to:
Health promotion and illness prevention services;
Primary care for minor or episodic illnesses;
Chronic disease management;
Management and coordination of patient care across the spectrum of care (i.e. referral to specialists and other providers, case management, case conferences and acting upon consultative advice);
Primary reproductive care;
Cultural safe care for all patients but especially aboriginal patients;
Primary mental health and substance use (MHSU) services;
Support for the terminally ill;
Coordination and access to rehabilitation;
Multidisciplinary team planning for the ongoing health needs of patients.
The ideal candidate is a self-motivated leader who takes initiative, is comfortable with ambiguity, and has the following skills and experience:
Current provisional/practicing registration as a Nurse Practitioner with the BC College of Nursing Professionals (BCCNP);
Relevant clinical nursing experience, including clinical knowledge and real passion for providing quality, compassionate care to motivate and inspire others.;
Strong understanding of provision of culturally safe care for patients;
