The program has an opening for a regular Sunday physician starting as soon as possible.
The EDiCare program is a team-based Geriatrics program implemented in the Vancouver General Hospital Emergency Department. The mandate of EDiCare follows the “Home is Best” philosophy for the Frail Elderly. Its goal is to redirect frail elders from a path of hospital admission towards discharge home when it can be safely facilitated with community and medical supports.
The EDiCare physician works as a member of this unique interdisciplinary team which includes Nursing, Care Management Leaders & Transition Services Team members. As a team, Frail Elders in the Emergency Department are identified and reviewed for appropriateness and potential for a safe discharge home rather than a likely prolonged hospitalization.
To ensure successful discharges from the Emergency Department, the EDiCare physician is responsible for developing and implementing safe discharge plans in collaboration with acute care services, community primary care physicians and community/Home Health services.
Key Accountabilities:
• Provide patient and family centred care
• Support the Emergency Department in managing complex vulnerable Geriatric patients, to avoid hospital admission when appropriate
• Develop collaborative trusting professional relationships with interdisciplinary team members, health authority staff and community physicians.
• Liaise with relevant community primary care providers and specialists to support safe discharge plans in the Frail Elderly
• Attend EDiCare rounds and /or huddles
• Communicate clearly with patients, families and other health care providers.
• Initiate and engage with patients and family members in challenging Goals of Care discussions
• Provide medical oversight for home health nurses providing care for patients in the community
• Partner with Home and Community Health to promote team-based care to prevent unnecessary ER visits and admissions.
• Initiate physician to physician communication when necessary for furthering program objectives.
Required Qualifications:
MD or equivalent medical degree
College of Family Physicians of Canada Certification (CCFP) or equivalent certification.
Additional experience or education working with geriatric patients
Experience working in an interdisciplinary team environment
Competencies:
• Geriatric interest and or training
• Experience in joint care planning
• Believe in “Home is Best” philosophy (model of care)
• Aware of community resources
• Excellent communication skills
• Experience working as an interdisciplinary team member
