Santé Ouest Community Health Centre, a multidisciplinary team-based approach clinic in Kitsilano, is looking to have a Nurse Practitioner join their clinical team. It serves the francophone population of the Lower Mainland, aiming to provide primary, low-barrier, culturally safe, and coordinated care. The CHC offers longitudinal care (including mental health and addiction services). The patient population will consist of francophone newcomers, LGBTQ2S+ and Indigenous individuals, isolated seniors, youth, and other groups who have faced barriers accessing healthcare in the English-speaking primary care system. Very diverse population in terms of immigration status, ethnicities, gender, income levels, etc.
As part of a multi-disciplinary approach to full-service family medicine, an interested NP would work collaboratively in the team environment and start their own patient panel as per Ministry requirements.
This will be a full-time or part-time contract position. This clinic is excited to add to their team a Nurse Practitioner interested in working together to provide primary care for the surrounding community of patients. This initiative is part of Primary Care Networks (PCNs) evolving in Vancouver. NPs interested in this opportunity should be willing to work collaboratively and participate in PCN development in the community.
The clinic will adopt a holistic approach, and will conduct appointments both virtually and in person to promote care accessibility. There are 4 exam rooms and 1 charting room. The clinic’s multidisciplinary team will consist of 3 physicians, supported by 2 nurse practitioners, 2 registered nurses, 2 social workers and 3 medical office assistants.
On-call: Required (details to be discussed).
Language: French is required, any other languages are welcome.
Clinic Hours: Monday to Friday, 9am-5pm.
Parking/transit information: Underground pay parking and some free street parking, easily accessible by transit.
