Role and Facility Information

This is an exciting opportunity to take over a partial patient panel from a highly regarded family physician transitioning to retirement within the next year. You'll have the chance to work alongside them during the transition, ensuring continuity of care while building out your own panel.

As a Family Physician here, you will:

- Provide full-scope family medicine, from pediatrics to geriatrics.
- Engage in shared decision-making with patients and care team to promote holistic, patient-centered care.
- Collaborate with our interprofessional team to deliver integrated team-based primary care.
- Participate in community outreach and health promotion initiatives.
- Participate in shared after-hours on-call rotation every 9 to 10 weeks.
- Contribute to broader clinical activities, including quality improvement, clinical meetings, program development and other non-client specific activities requiring physician expertise.

- A seamless transition into an existing panel – Inherit 250-500 patients from a highly regarded retiring physician, with a gradual handover to ensure continuity of care.

- A modern, well-equipped space – Work in a fully equipped clinic featuring eight exam rooms, a collaborative clinician workspace, dedicated virtual care offices, and an on-site pharmacy.

- Full-scope family medicine
- On-site dental services
- Pharmacy support
- A parent support program for families with children (ages 0-3)
- Our primary care team includes seven Family Physicians, two Nurse Practitioners, two Registered Nurses, five Medical Office Assistants, and on-site clinical pharmacists, all working together to provide comprehensive patient-centered care.
