0.3 FTE Hybrid GP Opportunity – Cambie Gardens Complex Care Supported Housing

Vancouver Coastal Health is seeking a family physician to support residents at Cambie Gardens, a supported housing site for individuals with complex physical disabilities and medical needs.

Cambie Gardens is part of the redevelopment of the George Pearson Centre, with 44 residents transitioning into supported housing units embedded within a mixed residential complex. Residents have significant physical disabilities (many wheelchair-dependent, some ventilated or tube-fed) with complex medical needs including spinal cord injury, traumatic brain injury, and stroke. Some residents also experience co-occurring mental health and substance use challenges.

This role provides longitudinal primary care and episodic medical support in collaboration with embedded nursing staff who provide day-to-day monitoring and initiate clinical consultations.

Time Commitment 0.3 FTE Includes:

● Two in-person rounding sessions per week (typically Tuesday and Friday afternoons)

● Availability for consultation Monday–Friday, 9–5

● Virtual care and care coordination as needed

Physician Scope / Activities

Act as MRP for approximately 44 residents.

Provide primary care support for common clinical issues including:

● Pressure injuries and wound care

● UTIs (many residents have neurogenic bladder and catheterization)

● Respiratory infections in ventilated patients

● Neurogenic bowel / constipation management

● Medication management for agitation or behavioural symptoms

Other activities include:

● Responding to nursing consultations (virtual or in person)

● Coordinating care with specialists and community teams

● Medication management and care planning

● Supporting hospital transitions when needed

● Joining the call pool and being on call once every 4 weeks
