About the Job
The Registered Nurse, Hospital at Home:
• Provides professional and comprehensive hospital level nursing services to admitted patients in their home environments
• Completes patient assessments (virtual and in-person), assess and monitors the health status of assigned patients to identify, document and follow-up on patient status by performing duties such as:
• Collaborates with the nursing team, interdisciplinary team members, community care partners, and the patient and family to develop and modify a patient care plan, including the flow of care (e.g., transitions between hospital and home), to address the assessed needs of the individual by:
• Provides nursing care by performing nursing functions
• Evaluates patient response to care by assessing the effect of the nursing interventions as identified in the patient’s care plan and modifying interventions as appropriate. Initiates the interdisciplinary team evaluations of patient care, including readiness for discharge.
• Provides virtual care and remote patient monitoring in accordance with plan of care.
• Contributes to the ongoing development of the nursing department and/or the nursing unit
• Adheres to Interior Health policies, procedures, and guidelines to maintain a safe environment for patients and families.
• Maintains a safe environment for patients/family and Hospital at Home care team
• Participates and attends in-service and other educational programs as required. Identifies learning goals and maintains and updates current clinical competence and knowledge within area of practice.
• Acts as a preceptor or mentor to guide the clinical experience of assigned new nursing staff and students; provides input to the Manager as requested on individual’s progress in achieving established standards of care.

This is a Permanent Full Time opportunity.

Shifts of work will be scheduled days, evenings and nights rotating from 07:00 to 19:00, 11:00 to 23:00, 19:00 to 07:00 hours.

Construction of the six-storey, Centennial Patient Care Tower was completed in May 2012. It includes an expanded emergency department and ambulatory care department, five new operating rooms, a rooftop helipad, an expanded adult inpatient psychiatric unit, and extra space for more patient beds in the future. Kelowna General Hospital is the site of the brand new clinical academic campus for the University of BC Medical School.

Qualifications
• Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM)

• Three (3) years of experience including at least two (2) in acute care medical or surgical services within the last five (5) years required

• Basic Life Support, Level C certification required


• Valid Class 5 B.C. driver's license
