Role and Facility Information

Island Health is seeking General Practitioners to work with a team of primary care providers and other allied health staff to collectively deliver integrated team-based care to patients.

In this group service contract position the physician will work collaboratively with an inter-disciplinary health care team to provide culturally safe longitudinal and urgent primary care. Patient visits will consist of scheduled longitudinal and unscheduled urgent care or same day primary care patient visits. Services to be provided by the physician include but are not limited to:

• Longitudinal care and attachment to a diverse patient panel, with initial focus on patients with complex medical conditions

• Provision of 30-35% of urgent primary care services for injuries and illnesses that should be seen by a health care provider within 12 to 24 hours but do not require the level of service or expertise found in an emergency department.

• Health promotion and illness prevention services;

• Primary care for minor or episodic illnesses;

• Chronic disease management;

• Management and co-ordination of patient care across the spectrum of primary, secondary and tertiary care;

• Primary reproductive care;

• Primary mental health and substance use (MHSU) services

• Support for the terminally ill;

• Coordination and access to rehabilitation;

• Provide medical coordination and participate in multidisciplinary team planning for the ongoing health needs of patients;

• Assist in in-patient care at Saanich Peninsula Hospital, and

• Clinical administrative duties

As a group of primary care providers, the physician together with the other practitioners (physicians and nurse practitioners) will assist in supporting Saanich Peninsula Hospital inpatient care for their admitted patients. There will be opportunity to participate in a call group program supporting inpatient care.

Island Health looks forward to supporting primary care providers and staff at this alternative payment site to provide innovative primary care services that are at the forefront of primary care improvement in BC. Modeled after best practices, primary care providers will have the opportunity to optimize data from the clinic information system to improve patient care.

Part Time or Full Time (0.5-1FTE)

Hours of work:

The Centre operates 365 days per year during the hours of 08:00 – 20:00. Each practitioner will provide service during less desirable shifts (weekend, evening and holidays) in an equitable manner. Schedules are managed mutually amongst the primary care providers to ensure availability and continuity of care for patients.

After hours care for patients is inherent in the services. The physician will participate in an after-hours call group, sharing after-hours coverage with other primary care providers, to include physicians and nurse practitioners, at the clinic.

FFS for inpatient services at Saanich Peninsula Hospital

Required Qualifications:

• Eligible for licensure with the College of Physicians and Surgeons of British Columbia (CPSBC).

• Family Medicine Certification of the College of Family Physicians (or equivalent).

• Successful candidates will be required to obtain Associate or Active Privileges at Saanich Peninsula Hospital

Preferred Qualifications:

• Experienced primary care providers capable of supporting breadth of patient complexities, urgent care services, and inpatient MRP services
