Role and Facility Information

The Cowichan Valley Division of Family Practice Society is seeking family practitioners to work with a team of practitioners and other health providers to collectively deliver care to patients in the Central Duncan Area, working out of the CareBridge Clinic as part of the Cowichan Primary Care Network.

The CareBridge Clinic aims to provide attachment for community members in Cowichan who currently do not have a primary care provider. This position will provide regular primary care and will provide care both in the clinic and in the community as needed.

In this service contract position the Family Practitioner will participate as a member of an inter-professional team that will include Nurse Practitioners, other FPs, medical office assistants, registered nurses, social workers and behavioral health consultants.

The Family Practitioner will participate in an after-hours call group, sharing after-hours coverage with other practitioners at the clinic.

Benefits

May be eligible for additional remuneration in accordance with the Rural Subsidy Agreement (RSA), such as:

•Retention Premium = 4.2%

•Retention Flat Fee = $6,508.80

•Recruitment Incentive (pro-rated) $10,000

•Relocation Costs Reimbursement up to a maximum of $15,000

•CME:

- Years 3 & 4: $2,200/annum

- After 4 years: $4,400/annum

Preferred Qualifications

• Experience with medically complex patients experiencing mental health, comorbidity, and mobility challenges.

• Experience in team-based care

Required Qualifications

Eligible for licensure with the College of Physicians and Surgeons of British Columbia (CPSBC)

Eligible for Provisional Privileges at Cowichan District Hospital
