Subspecialty Geriatric Medicine

Role and Facility Information

The University Hospital is seeking a Geriatrician to provide Inpatient and Outpatient Geriatric Services at various sites. The successful applicant will become a member of the Dept of Internal Medicine, but will not be required to participate in the call rota.

Geriatric Inpatient and Outpatient services include but are not limited to:

- Assessment, treatment and follow-up in outpatient clinic, or in the hospital;

- Medication review and education;

- Referrals from family physicians or attending specialists for elderly patients living in the community or in a long term care facility;

- Referrals from physicians with hospitalized acute care elderly patients whose medical, functional or psychosocial problems are interfering with either their discharge or their ability to function in the community;

- Complex competency assessments in collaboration with psychiatry;

- Discharge planning;

- Geriatric medical consultation and diagnosis; and,

- Development of treatment and management strategies.

- Consultation with the community based multi-disciplinary integrated health team for seniors team including problem solving and care planning for elderly patients;

- Consultation with the patient’s family physician and family;

- Provision of consultation and clinical program enhancement activities including planning and evaluation of geriatric services and resources;

- Consultation with staff and other professionals to develop and deliver an effective and efficient geriatric services program; and,

- Participation in the evaluation of the efficiency, quality and delivery of the Services, including and without limiting the generality of the foregoing, participation in medical audits, peer and interdisciplinary reviews, chart reviews, and incident report reviews;

Performance Deliverables:

- New Outpatient Clinic (OPC) assessments;

- Follow up assessments (acute and OPC);

- Consults (acute and OPC);

- Outreach /Telehealth: Northern Health and Geriatrician agree to develop a plan to implement Outreach and Telehealth consultation services to support patients and ensure access to areas not served by existing Geriatric clinics
