Role and Facility Information

If you’re a physician who thrives in a collaborative, team-based environment and loves seeing patients succeed beyond the acute setting, this role offers real impact. You’ll lead care for patients in a sub-acute environment, guiding their transition from hospital to home, long term care, or other destinations. As the MRP, you’ll manage admissions, daily care, and discharge planning, working closely with an interdisciplinary team to ensure every patient receives safe, timely, connected care.

The work is hands-on and highly team-oriented: bedside rounds, structured planning with nursing and allied health, coordinating discharges, and supporting smooth weekend transitions. You’ll have the opportunity to shape patient journeys, reduce unnecessary transfers, and strengthen care plans that improve recovery, independence, and quality of life. Strong systems support and a daily rotating MRP model help ensure balance, shared accountability, and continuity for patients.

Alongside direct care, you’ll contribute to the broader quality of the unit -- reviewing referrals, participating in team rounds and case conferences, providing consultative guidance, and helping drive quality improvement, medical standards, and best practices. For those interested, there is also room to take on a medical leadership role within the PATH unit.
