Role and Facility Information

The Lookout Clinic (Surrey North Community Health Center) is a Fraser Health-funded clinic operated by Lookout Housing and Health Society. The clinic has had a strong, collaborative arrangement with Fraser Health since its inception in 1992, establishing sustainable, wraparound public health programs in parallel with primary care for the population we serve in North Surrey (Whalley). These programs include screening and treatment of STIs, HIV, and HCV, as well as wound care, addiction medicine and longitudinal health care for chronic disease management. Working in supportive and trauma-informed team-based care setting (NPs, FPs, RNs), you will be able to provide services that are making a difference in addressing primary care access and health equity. In any given day at the clinic you may find yourself providing spectrum care for OUD, (including standard OAT, iOAT or the fentanyl patch) alongside addressing the primary care needs. It's truly unique and fulfilling work.

We are looking for 1.0 or 0.5 FTE Primary Care Network Nurse Practitioner Contract with a reduced panel commitment to fit the needs of the clinic's population to join as a full-time regular staff member.
