Specialty Nurse Practitioner

Role and Facility Information

The Sunshine Coast Primary Care Network (PCN) is seeking a part-time (0.5 FTE) Nurse Practitioner to provide comprehensive, longitudinal primary care on the Sunshine Coast. Successful candidates will be matched with a clinic based on community need and mutual fit and will join a collaborative, team-based practice.

Nurse Practitioners have the opportunity to develop a broad scope of practice, caring for patients across the lifespan, including those with complex chronic conditions, older adults, and individuals requiring palliative care. Depending on practice location and community need, there may also be opportunities to develop clinical interests such as mental health and substance use, addiction medicine, gender-affirming care, sports medicine, or other areas of specialized practice.

The Sunshine Coast PCN is committed to delivering culturally safe, patient-centred care. Nurse Practitioners work alongside family physicians and have access to a network of health professionals, including registered nurses, a social worker, pharmacist, dietitian, and counsellors. Together, the team provides comprehensive, high-quality primary care and works to improve access and health outcomes for patients.

Key Responsibilities:
- Provide comprehensive, patient-centred primary care including assessment, diagnosis, treatment, and ongoing management of health conditions.
- Assume and progressively build a patient panel of approximately 400 patients in Year 1, 650 patients in Year 2, and 800 patients in Year 3 (per 1.0 FTE).
- Conduct comprehensive medical histories and physical assessments, and develop individualized care plans in partnership with patients.
- Order, perform, and interpret appropriate diagnostic tests, including laboratory investigations and medical imaging.
- Diagnose and manage acute and chronic health conditions and provide ongoing follow-up and monitoring.
- Prescribe medications and therapies, and provide orders and direction to other health-care providers within scope of practice.
- Complete medical and functional documentation, including disability, ICBC, WorkSafeBC, and extended health benefit forms.
- Perform minor procedures within scope of practice, such as IUD insertions, suturing, and biopsies.
- Refer patients to medical specialists and other health-care services as clinically appropriate, and coordinate care across providers and services.
- Provide health education, counselling, preventive care, and support for healthy lifestyle choices and self-management.
- Collaborate with physicians, nurses, pharmacists, counsellors, and other health-care professionals to provide coordinated, team-based care and improve patient outcomes.
