Change lives and create forever skills in teens and young adults. Bring your clinical expertise and passion to a rewarding role as a Nurse Practitioner within the Primary Care Network (PCN) at the Chilliwack Youth Health Centre (CYHC).
We are seeking a Nurse Practitioner with an interest in working with physicians and our partner agencies to break down barriers to care. We anticipate the position will involve outreach with multi-barriered youth, including homeless youth, youth with mental health and substance use challenges, and Indigenous youth. The Nurse Practitioner will provide care, leadership, and expertise, per CYHC policies. They will be supported by a caring and skilled team of family physicians, psychiatrist and counseling staff. Specifically, the Nurse Practitioner will ensure continuity in primary care services at all three sites. There is an opportunity for online/on-phone as well as in-person care.
Located in Chilliwack, the CYHC provides care to young individuals between the ages of 12-26. The CYHC collaborates with a distributed model, with service at three sites:
• Neighborhood Learning Centre (main site, attached to Chilliwack Secondary School), open three afternoons (1 -7 pm)
• Stó:lō Primary Health Care Centre
• Tzeachten University of the Fraser Valley (UFV), Chilliwack Campus
The CYHC provides mental, physical, and sexual health care through a team of counsellors, family physicians, and nurse practitioners. CYHC‘s counselling service is provided by Master level trainees, supervised by doctoral students in Psychology, and overseen by a registered psychologist. In addition to one-to-one care, CYHC offers two Cognitive Behavior Therapy groups, a group focused on mild to moderate anxiety (YMinds) and a trans care group. CYHC also works closely with the Ministry of Social Development and Poverty Reduction to support young people transitioning to adulthood and in need of income support.
As a training site, the CYHC offers multiple in-service educational opportunities. The Nurse Practitioner will work closely with counseling interns from a variety of Universities; The University of British Columbia, Simon Fraser University, Trinity Western University, City University (U), Adler U, Fielding U, Yorkville U, Dalhousie U and UFV.

• Current provisional/practicing registration as a Nurse Practitioner with the BC College of Nurses and Midwives.
• Relevant, clinical nursing experience.
