In Maple Ridge and Pitt Meadows, more than 10,000 residents are in need of a primary care provider, many with complex care needs. Based on a team-based model of care, this brand new, community hub will foster patient attachment and access to services prioritized by local physicians. The Ridge Meadows Wellness Centre will co-locate the following clinics and services: - an urgent and primary care centre - a primary care clinic - rehab and education spaces - allied health and specialist services The Ridge Meadows Wellness and Urgent and Primary Care Centre is the first Primary Care Network centre in BC designed through a collaborative decision-making process between local physicians, the health authority and partner organizations. A DIFFERENT COMPENSATION MODEL Looking for some different than fee-for-service? The Alternative Payment Plan contract offers income stability to help navigate uncertain global health conditions. Our reasonable patient attachment goals will allow you the flexibility to explore other service options, such as working as a hospitalist, or simply enjoy a more relaxed work/life balance. As well, being employed by the health authority instead of as independent contractor will allow you the freedom to focus exclusively on the work you love to do, and for what brought you into the medical profession in the first place: patient care. TEAM-BASED CARE: OPTIMIZE YOUR SCOPE Work as part of an interdisciplinary team so that you can connect your patients to the service/s they need (at no cost to them) and the burden of care will not rest solely on your shoulders. The team of specialist services and allied health professionals that will be working to support you and your patients will include: - seniors services: geriatricians, a seniors outreach coordinator, a geriatric social worker and a geriatric registered nurse. - chronic pain services: a pain specialist, an occupational therapist, a physiotherapist and a registered nurse. - integrated chronic disease services: an occupational therapist, a physiotherapist, a social worker, dietitian and registered nurse. -adult mental health services: a psychiatrist, a clinical counsellor and a mental health nurse. The allied health team includes a social worker, a cultural advisor, a cultural navigator, pharmacists, a maternal/child registered nurse, and a child and youth counsellor. Plans are underway to expand the team to include a pharmacist, an occupational therapist, a dietician and a maternal nurse. MAKE AN IMPACT With the support of our Primary Care Network service team, you will be supported with the resources and services to attach the region’s most vulnerable populations, including seniors, adults with chronic pain, chronic disease, mental health and substance use challenges, children and youth with mental health and substance use challenges and maternity. SHARE YOUR PANEL Interested in trying the team-based model but still want the freedom to engage in other practice options? Explore practicing in innovative ways with part-time options that will allow you to share your patient panel with other general practitioners and nurse practitioners based in the clinic. SUBSPECIALTY DEVELOPMENT Do you have a passion for supporting seniors, adult mental health, child and youth mental health and substance use, chronic disease or chronic pain? Specialists, including a psychiatrist, pain specialist and geriatricians will be available on a rotating basis to provide consultation support to Ridge Meadows physicians. This support will position you to develop a subspecialty in these areas. As a new-to-our-community physician, you will be invited to enjoy a mentorship program, where you will be matched with a seasoned family physician to help you get orientated and established. The Division will also support you over the long term by hosting local CME sessions, organizing social events such as our summer BBQ, advocating for your family practice, or facilitating physician-led patient care improvement initiatives that engage the entire medical community. For example, during the first two months of the COVID-19 pandemic, the Division engaged over 500 physicians, nurse practitioners, specialists, MOAs and community partners in virtual meetings to develop a united, collaborative and cohesive local response to the health crisis, resulting in such pioneering initiatives as a temporary "pop-up" clinic designed to provide a safe space for in-person patient care during COVID-19. “The Ridge Meadows Division has been a leader in the province when it comes to including specialists at the table. Other supports and opportunities in our medical community include: - Ridge Meadows hospital, a full-service hospital that provides 24/7 emergency care, surgery, maternity, critical care, a new MRI department, and a host of other services. - A vibrant community of approximately 40 specialists who regularly collaborate with family physicians through patient care improvement initiatives. - A child and youth mental health and substance use support centre called Foundry. - Physician networks designed to foster subspecialties in adult mental health and addictions, women’s health, male health, chronic pain, seniors, maternity, children and youth, and end of life. Ridge Meadows is also home to two Primary Care Networks, part of a new, robust and integrated system of care launching across BC to increase patient attachment and access to quality primary health care that effectively meets their needs.
