Role and Facility Information
The North Shore Palliative Care team is a full scope Palliative Care team looking after patients in the communities of North and West Vancouver in association with Lions Gate Hospital (LGH) and Vancouver Coastal Health. We are a rapidly growing team looking after patients on the North Shore and the Sea-to-Sky Corridor. Our focus is to improve quality of life in patients with life-limiting illnesses. We are a teaching hospital in affiliation with the University of British Columbia, Family Medicine and Palliative Care programs.
We provide care in the following venues:
1. Acute/tertiary care (15 bed Palliative Care Unit at LGH) - We have a specialized multidisciplinary team on our Palliative Care Unit. We are integrated with our medical oncological colleagues, internists, subspecialists, surgeons and interventional radiologists to provide robust, evidence-based care. We provide admitting (MRP) and consultative services. We also provide education and advocacy for Palliative Care to our hospital-based colleagues as well as those in the community. The successful candidate will share in consultation and on call coverage for both inpatients and community patients.
2. Ambulatory care clinic – A stand-alone clinic based in hospice focused on palliative care for patient with a wide range of life-limiting illnesses. We provide primary and shared palliative care for ambulant patients. We work in conjunction with a multidisciplinary team in the community.
3. Community care - We work in conjunction with the team of palliative nurse practitioners, palliative care resource nurses, home care nurses, SW, OT, PT and wound care. We provide shared palliative care to help support symptom management and death in the patient home.
4. North Shore Hospice (15 beds) - Admission to hospice is via the palliative care physicians only. We have a multidisciplinary team that supports patients and their loved ones including nurses, social workers, spiritual care, and music therapy. Criteria for admission is prognosis less than 3 months of life, symptom management needs that cannot be met at home, and patient is a North Shore resident. We do make compassionate exemptions on a case-by-case basis.
Excellent communication skills and a commitment to an inter-professional care model are mandatory.
