Role and Facility Information

The Éyameth’ Health Centre will offer safe, accessible and culturally appropriate primary care services to all residents North of the Fraser River between Agassiz and Mission.

The vision for Éyameth’ includes a medical clinic housed with a center of excellence, weaving together innovative, transformative health and wellness services that are driven by the community to address health care gaps in the region.

The Éyameth’ Health Center is a society-run, Nations-led Primary Care Center under the provincial First Nations Primary Care Initiative. We are a groundbreaking, standalone primary care facility partnered with the Ministry of Health, Fraser Health, FNHA and the Chilliwack Division of Family Practice.

Éyameth’ Health Centre is located at 46090 Lougheed Highway and will open in Summer 2025.

Éyameth’ Health Centre services are designed to provide wraparound, team-based primary care, mental health and traditional wellness services for all residents of the North Fraser region. There will also be a home hemodialysis unit, pharmacy, level 1 lab and rehabilitation room. It is expected that the Centre will initially operate Monday to Friday (excluding Statutory Holidays) during regular business hours (this may change over time due to operational needs).
The Nurse Practitioner is expected to collaborate in team-based care for patients and support the practice of Western and Traditional Wellness approaches to patient health. A minimum panel size is expected to reach 1400 patients by March 2027. Estimated Yearly Remuneration for the current service contract compensation is $171,533 - $182,968 over 3 years based on panel attachment. Applicants may be eligible for additional incentives.

RESPONSIBILITIES
The Nurse Practitioner (NP) will be responsible for practicing with cultural humility, providing primary care in a culturally safe, team-based environment that recognizes the physical, mental, emotional, and spiritual aspects of the client (and family) to provide whole person care, in compliance with policies and procedures of acceptable BC health and wellness regulations and standards.

Core Services in compliance with policies and procedures of acceptable BC health and wellness regulations and standards:
1. Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
2. Recent relevant clinical nursing experience is considered an asset.
3. Culturally safe care integrating cultural humility, traditional values and ways of being for patients.
4. Serves as a team member with others such as General Practitioners and allied health staff as well as traditional wellness advisors who collectively provide primary care to the patient.
5. Ability to communicate and collaborate with patients/clients/residents and families about health findings, diagnosis, treatment, self-care and prognosis.
6. Diagnoses and treats previously undiagnosed patients/clients/residents for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner's scope of practice; writes orders for treatment and medications; provides first line care in emergencies.
7. Monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment.
8. Primary mental health and substance use (MHSU) services including assessment, diagnosis, individualized care plans, early support and referral.
9. Consultation and referral to other health care professionals where beneficial.
10. Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
11. Patient education, preventive care and assessment.
12. A wide range of collaboration with other interdisciplinary team members such as huddles, conferences, consultations, team meetings, service co-ordination and referrals to other wellness and health care providers.
13. Ability to self-assess performance and assume responsibility and accountability for own professional development, educational or consultative assistance when appropriate.
14. Must have a valid class 5 driver’s license and reliable transportation
15. You are passionate about supporting First Nations and all individuals and families on their health and wellness journeys.
16. You have training in Cultural Safety & Humility/willing to take ongoing training.
17. Interest in mentorship, teaching and innovative health and wellness service delivery in a community-driven and team-based environment.

Type of Position: Full-time position preferred, however part-time negotiable. This position is subject to a 3-month probationary period and performance evaluation. Contract up to a 1.0 FTE (typically 1680 hours a year)
