Overview:
Join our dynamic team at Harbour Health Centre as a Nurse Practitioner specializing in primary care network services. This role offers a unique opportunity to provide comprehensive healthcare services to patients within the beautiful, rural community, emphasizing preventative care, health promotion, and chronic disease management. As a vital member of our interdisciplinary team, you will collaborate with physicians, allied health professionals, and community resources to deliver patient centered care. This is an incredible opportunity to live and work in a mild climate with abundant natural, oceanfront beauty and outdoor activities (hiking, skiing, kayaking, whale watching, biking, camping and more). A $5,000 bonus incentive is offered for new NPs as well as free or subsidized accommodation to support relocation.
Key Responsibilities:
1. Building a healthy community is at the heart of this role. Working with a multidisciplinary team that includes a nursing clinic, social worker, dietician, diabetes and chronic disease management programs, chiropractic, dentists, mental health, women’s and youth sexual health clinics and supportive and preventative programs, the NP acts as a central provider of care on the patient and caregiver’s team.
2. Conduct comprehensive health assessments, including physical exams, medical histories, and diagnostic tests, to formulate accurate diagnoses and develop individualized treatment plans.
3. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care outcomes and programs effectiveness.
4. Maintain accurate and up-to-date electronic medical records, ensuring compliance with regulatory standards and confidentiality protocols.
5. Engage in ongoing professional development activities to enhance clinical knowledge and skills, staying abreast of current evidence-based practices and healthcare trends.
6. Manage acute and chronic illnesses, prescribe medications, and initiate referrals to specialists or other healthcare professionals as necessary.
7. Provide patient education and counselling on health promotion, disease prevention, and self-management strategies.
8. Provide full scope primary health care services including health promotion and illness prevention, primary care for minor or episodic illness, chronic disease management, management and coordination of patient care, primary reproductive care, primary mental health and substance use (MHSU) services, support of the terminally ill, coordination and access to rehabilitation; support for hospital, home, rehab and long-term care facilities where permitted or appropriate, multi-disciplinary team planning, clinical administrative services, both in the patients home, in PHHC, hospital or other facility.
9. Each 0.5 FTE nurse practitioner will assume a patient panel of 200 patients in year 1, 325 patients in year 2, and 400 patients in year 3. PHHC NP spaces have state-of-the-art equipment to support their practice. Located approximately 30 minutes from the local hospital in Sechelt, BC.
Qualifications:
• Master’s degree in nursing with specialization as a Nurse Practitioner
• Current registration and licensure with the British Columbia College of Nurses and Midwives (BCCNM)
• Certification as a Nurse Practitioner (NP) from a recognized regulatory body
• Proficiency in utilizing electronic medical records (EMR) and other healthcare technologies.
• Strong interpersonal and communication skills, with the ability to collaborate effectively within an interdisciplinary team.
• Commitment to providing patient-centered care and promoting health equity within the community.
Benefits:
• Competitive salary. The salary for a part-time (0.5 FTE) PCN NP is $85,766 (Yr 1); $88,625 (Yr 2); $91,484 (Yr 3).
• Continuing professional development funds of $5000, licensing fees coverage, can be accessed through NNPBC program under NP Funding.
• An overhead amount of $37,500 per 0.5 FTE is provided through the PCN for overhead or professional development costs associated with working at PHHC.
• Practice sharing and sub-contracted or locum coverage options are supported through PHHC recruitment.

• $6500 annual incentive for the first three years for attaining Patient Attachment System targets and for Quality Improvement.
