The Coast Mountain PRIMARY CARE NETWORK (PCN), a collaborative partnership of the Pacific Northwest Division of Family Practice, local First Nations, the First Nations Health Authority, and Northern Health, invites application from Nurse Practitioner for flexible opportunities (part or full time) based out of the following clinics

Nurse Practitioners will have the opportunity in Terrace to practice full scope family practice, as well opportunities to pursue other Nurse Practitioner areas of interest through the Coast Mountain Primary Care Network such as primary care pediatrics, or low barrier clinic supports. Applicants moving through to the interview phase will receive an interview at each of the above listed clinics.
Supported by a comprehensive team of primary care network allied health care providers, the Coast Mountain Primary Care Network will offer comprehensive, holistic, patient-centered, culturally safe care to individuals and families living in Terrace and the surrounding communities.
The following Allied Health Professionals are planned to support to the Coast Mountain Primary Care Network: Indigenous wellness providers, psychologists, physiotherapists, occupational therapists, dietitians, kinesiologists, primary care nurses, and pharmacists.
Terrace is a spectacular mountainous community, situated along the awe-inspiring Skeena River, with the beautiful, culturally rich communities of Kitselas and Kitsumkalum located a short drive from the city.
The Coast Mountain region is surrounded by snow capped, stunning mountains, sparkling lakes with sandy beaches, and world class outdoor activities including mountain biking, hiking, kayaking, fishing, cross country and alpine downhill skiing, all within 20 minutes from Terrace. A lively arts scene brings local concerts and talented artisans together, along with international visitors.
About Primary Care Networks (PCNs):
A PCN is a clinical network of local primary care service providers located in a geographical area, with patient medical homes (PMHs) as the foundation. A PCN is enabled by a partnership between the local division of family practice and health authority, along with local First Nations.
In a PCN, physicians (via patient medical homes), other primary care providers, allied health care providers, health authority service providers, and community organizations work together to provide all the primary care services a local population requires. Together, they:
• Enhance patient care using a team-based approach to care.
• Support each other and work to their strengths.
• Further link patients to other parts of the system, including the health authority’s specialized community services programs for vulnerable patient groups (e.g., frail elderly, mental health and substance use).
• Collectively increase a community’s capacity to provide greater access to primary care for people without a primary care provider.
Nurse Practitioner Coast Mountain Primary Care Network

Position Posting:
Practicing collaborative, culturally safe and inclusive care, the Nurse Practitioner is responsible and accountable for the comprehensive assessment of patients/clients/residents including diagnosing diseases, disorders, and conditions. The Nurse Practitioner initiates treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits, and conditions, and employer policies and procedures. The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary health care team, making referrals to Specialist Physicians and others as appropriate.
This position collaborates with Family Physicians, patients/clients/residents and other health professionals to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence-based strategies to improve health and participates in policy-making activities that influence health services and practices. This position participates in peer review and self review to evaluate the outcome of services at the patient/client/resident, community and population level.
Key Areas of Responsibility
- Diagnoses and treats previously undiagnosed patients/clients/residents for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner’s scope of practice; writes orders for treatment and medications; provides first line care in emergencies.
- Monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment.
- Establishes priorities for management of health, diseases, disorders, and conditions; provides follow-up treatment; communicates with patients/clients/residents and families about health findings, diagnoses and priorities, outcomes and prognoses; supports and counsels patients/clients/residents in their responses to diseases, disorders and conditions.
- Collaborates and consults with Family Physicians or other health care and social service providers as appropriate to assess and diagnose patient/client/resident status. Develops and implements longitudinal treatment plans. May admit and discharge patients/clients/residents to facilities according to organizational policies.
- Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
- Assigns work to other nursing and health care personnel; evaluates work and provides education and supervision as necessary; hires staff and evaluates performance.
- Participates in research contributing to improved patient/client/resident care and advances in nursing, health policy development and population health.
- Maintains population health focus by implementing screening and health promotion activities for populations at risk.
- Participates in interdisciplinary staff and nursing education through case presentations, mentoring, role modeling and facilitating the exchange of knowledge in the classroom, the clinical setting and the community; fosters primary care network health care partnerships.
- Develops, implements and evaluates policies and procedures related to nursing, interdisciplinary care and health system practices.
- Performs other duties as required.
Education and Experience
- Completion of BCCNM approved Master’s degree level Nurse Practitioner program.
- Current active full practice registration as a Nurse Practitioner with British Columbia College of Nurses and Midwives (BCCNM).
This position is subject to a current Criminal Record Check, including Vulnerable Sector Registry Check.
