Bring your clinical expertise and passion for compassionate care to a meaningful primary care role serving frail, home-bound older adults in Langley, B.C. We are seeking a dedicated Nurse Practitioner (NP) to step into a rewarding role in Geriatric Home-Based Primary Care.
About the Role
As a Nurse Practitioner in this primary care position, you will:
Collaborate with the Langley Seniors Clinic and other community health services to deliver comprehensive medical care to older adults with complex health needs.
Develop and manage individualized care plans focused on chronic disease management, medication optimization, and preventive health.
Work within a multidisciplinary team to support safe, effective, and timely care transitions.
Advocate for patients and families, helping them navigate health systems and access appropriate resources.
Contribute to quality improvement initiatives and the advancement of best practices in geriatric care.
