Salmon Arm Senior’s Health & Wellness Centre:
The Salmon Arm Senior’s Health and Wellness Centre provides services designed to support the health and wellness of seniors livings with chronic conditions. The goal of the program is to improve seniors care through timely access to specialized teams of healthcare practitioners who will provide a comprehensive geriatric assessment, intervention, treatment and follow-up care; focusing on independence, health promotion and prevention, stabilization and improvement of seniors’ health in the community.

Services offered include:
•Collaboration with Clients existing community support network (e.g. family physician / NP, Home and Community Care Case Manager) as well as linking client and family with services and resources (e.g. home and safety assessments; financial assessments; nutritional assessments; coordination and collaboration with home support)
•Comprehensive Geriatric Assessment, customized inter-professional care planning with the client and the family, personalized interventions and short-term follow-up and transitioning to community service.

Practice Details:
The Salmon Arm Senior’s Health and Wellness Centre (SHWC) is seeking a Geriatrician to join their dynamic team!

This position works collaboratively with all members of the patient care team to provide supportive quality care to the SHWC patients and families through geriatric assessments, follow up visits, medication adjustments, and patient education. Services may be offered both in-person or virtually.

The ideal candidate will have a passion for senior’s care, a strong understanding of the unique needs of seniors and their families, and the ability to thrive in an interdisciplinary team environment.

Responsibilities include:

Direct Patient Care Services:
•Provides a geriatric consultation service to clients at the Seniors Health and Wellness Centre (SHWC), upon referral by the MRP, when the consultation is not billed to MSP Fee for Service.
•Provides follow-up visits via videoconference, phone or in-person if not billed to FFS (must ensure that appropriate documentation regarding visit occurs even if not on-site at the time of follow-up).
•Gathers patient collateral information when it cannot be billed under FFS.
•Provides care to a patient who does not have valid MSP care card when it is appropriate.
•Advises and educates the client and the client’s family on diagnosis, treatment and prevention of disease and disability in the older adult when not billed to FFS.
•May provide home visits for assessment or follow-up if deemed necessary and schedule allows and when not billed to FFS.
•Any other patient specific clinical services that are not billed under FFS as appropriate.

Indirect Patient Care Services:
•Assists with the coordination of geriatric medical service for the SHWC, in collaboration with the MRP and SHWC Team when not billed to FFS.
•Contributes to and participates in the development and delivery of health care education for clients, clients’ families and program staff regarding specific geriatrics issues, including but not limited to continence, falls assessment and prevention, cognition, medication management, functional mobility, nutrition/hydration, elimination, pain and psychosocial concerns when not billed to FFS.
•Patient conferences or interviews that are specific to a patient but do not qualify for FFS billing.
•Phone call or follow-up with patient caregivers related to patient care plan, discharge plan or other conversation necessary for quality patient care when not billable to FFS.
•Working as a member of the multidisciplinary team in providing a coordinated service to patients including liaisons between the client’s general practitioner and Community programs and patient care related communication with team members. This may include all forms of electronic and verbal communication such as email, fax, text, face to face and telephone when not billed to FFS.
•Prescription renewals or calls to/from pharmacy to clarify an order when not part of a patient visit and therefore not billed to FFS.
•Patient specific case management time not billable to FFS (provide PHN if able).
•Clinical, patient specific advice related to patients not currently under physician’s care when SSC FFS codes do not apply and not otherwise billed to FFS. (Provide PHN if able)
•Completing patient care related forms that are not billable under FFS (provide PHN).
•Patient no shows as long as physician remains available on-site to support the program during this scheduled time. (Note records should be kept of each such patient no show, the date and time charged for the no show. The Agency and / or Ministry of Health auditors may require copies of these records to understand reasons and volumes.)
•Specific instruction to staff or medical students/ residents as part of patient’s care where necessary to ensure safe, quality care when it’s provided concurrent with patient care or in support of a patient specific care plan and not billed under FFS.
•Patient specific, clinically related research or evaluation.
•Patient specific quality improvement activities.

Clinical Administrative Services:
•Provides information to physicians and other care providers with the goal of increasing their ability to manage persons with geriatric syndromes when not billed to FFS.
•Supports the triple aim initiative of: Improving the patient experience of care (including quality and satisfaction); improving the health of populations; and reducing the per capita cost of health care.
•Serves as a member of the SHWC team, providing medical input/service to this program.
•Provides staff education and develops a culture of health promotion and prevention; develops resource materials for distribution to staff and physicians with respect to Geriatric Medicine.
•Recommends medical standards, policies and procedures for the program, taking into account regional, provincial and national medical standards and national accreditation standards.
