Role and Facility Information

Bring your clinical expertise and passion for providing quality and compassionate care to a challenging and rewarding role as a Nurse Practitioner. The Patient Assessment and Transition to Home (PATH) unit is an inpatient hospital-based program in Fraser Health where Nurse Practitioners assume the role of the Most Responsible Provider (MRP) for patients. As part of this rewarding MRP role you will have the opportunity to participate in compensated on-call work rotations to assist the unit remain open 24/7, 365 days a year. NPs in PATH units support and empower medically stable alternative level of care (ALC) designated patients with complex discharge requirements and facilitate successful transitions home to thrive in the community. 

This is your opportunity to:
Develop an understanding of various ALC designated patients and their needs.
Be proficient in complex assessment, diagnosis, and treatment/management planning.
Be responsible and accountable assuming the MRP role for inpatients

Specifically, you will gain experience in:
Complex discharge planning and advanced clinical decision making for patients with multiple comorbidities, chronic diseases, and variable social needs.
Principles of geriatric medical care to a defined patient population
Working in a team-based environment with a specialized discharge planning team to minimize hospitalizations, prevent health deterioration, and improve quality of life
Referral and consultation to and with specialists and in accessing community resources.

This is a great opportunity for a new Nurse Practitioner to join a well supported fellowship program to help consolidate clinical skills. Be part of our clinical enhancement program with regular in-house skills development such as suturing and x-ray interpretation in addition to receiving centralized NP orientation. You will be welcomed into a New Graduate NP Community of Practice team helping to support your successful transition into independent practice.

This position will also have a new and exciting postgraduate up to 6 month nurse practitioner residency program. FH is the first health authority to implement a formal residency program for NPs in the province, which will be key to supporting the development of NPs and practice-readiness in select programs. You will receive in depth in house advanced training within a clinical area relevant to the position, improve your team collaboration and develop a model of patient centred care.  This is your opportunity to consolidate your nurse practitioner education with a team helping to support your successful transition into independent practice.

During your residency, you’ll receive:
Mentorship in multiple clinical areas
Dedicated clinical supervision during clinical placements
Protected time for self directed learning relevant to your clinical practice
Coaching from a consistent experienced nurse practitioner throughout your residency
Individualized support in transitioning to independent provider
