The inclusion of the NP into the team is of great benefit for managing this population due to their needs for integration of care across disciplines and continuity of care. The Nurse Practitioner is one of the main points of contact that builds the plan with the physicians, patients and allied team members. These patients have complex medical needs and will require in-patient monitoring, co-ordination of care needs, education and optimization of therapy. Registered nurses, social workers, Dietitian, Pharmacist are some of the other integral team members the NP will be working with for the coordinated care of these patients.
