Opening in September, our new clinic in Burnaby is dedicated to providing primary care to four priority populations: newcomers, individuals with complex mental health issues, persons experiencing homelessness, and new mothers and babies. We are seeking passionate primary care providers committed to delivering comprehensive, compassionate care in a supportive environment.

• Address: #306 - 7366 Edmonds Street, Burnaby, BC V3N 1A9
• Facility: Burnaby Edmonds Centre for Healthy Communities - Health and Wellness Services
• Proximity: Adjacent to the PCN Allied Health team and across from Fraser Health's UPCC and New Canadian Clinic.
• Parking: Free parking available with parkade access on the same level as the clinic
• Bike Racks: Available near the Centre’s entrance on the 3rd level of the parkade
• Transit: Close to Edmonds SkyTrain Station and buses on Kingsway
Facilities:
• 8 exam rooms (including 1 with a bariatric bed and lift)
• 3 consult rooms
• Touchdown space for clinicians
Responsibilities:
• Provide high-quality primary care to diverse patient populations.
• Collaborate with a multidisciplinary team to ensure wrap around care.
• Engage in trauma and violence informed longitudinal primary care services.
• Adhere to the principles of a Patient Medical Home, ensuring patient-centered care.
• Coordinate care with specialized health services, hospitals, and other community services.
